Charting/cartography was a factor. Cartography is an ever-evolving art form; the challenge is to represent complex technical procedures in a manner that is easily interpreted by a crew in a busy cockpit, in poor lighting, etc. Charting issues can range from omitted or obsolete information to design issues.
